In order to apply for Housing and/or Support, please read our  procedure and information about the services we provide . Please then return the following forms to the relevant Area NAP Administrator by using the address at the bottom of the forms. 

For Kent and Medway services;

For Bexley and Greenwich services;

For Bexley services (including Sidcup, Welling, Belvedere, Erith, and Crayford), the range of support we offer is based on individual assessment carried out by our team leaders in conjunction with a joint assessment from the Bexley learning disability team based at Stuart House in Sidcup.

To start the referral process individuals will need to be on the Bexley learning disability register so please complete the information sheet below and return it to our Bexley area office to be considered. Each referral will need to meet the statutory  Fair Access to Care Services  criteria.
